Eligibility Criteria

Entry Guidelines

The Navigation Center welcomes any individuals who may experience mental or behavioral health issues, active addiction and substance use, and/or extensive criminal histories. However, only individuals that are capable of self-regulation and re-direction can be accommodated. Individuals must possess comprehension of program policies and expectations upon intake and during the duration of their stay at the Navigation Center.

Clients must be able to complete their ADLs independently. That includes: using the bathroom, eating, getting dressed, and controlling mobility. We are unable to support individuals with incontinence issues. The Navigation Center is not contracted to provide medical support to care for the above needs, residents are expected to function independently.

The Navigation Center is a Lane County Project. We are only authorized to accept referrals directly from Lane County Coordinated Outreach. Referrals must have completed a Front Door Assessment and have been added to Lane County’s Centralized Waitlist.

To complete a Front Door Assessment, please reach out to one of our community partners below.
